Below, you will find all of the required paperwork available in PDF format. Simply click on the file and it will open in a new window. Print the Intake Paperwork, complete it, and get it back to me by scan/email, fax, regular mail, or drop it by the office in person. Please read the HIPAA document and the Service Agreement. The signature page of the Service Agreement is included in the Intake Paperwork for your convenience.
Once I have your completed paperwork we will call you to schedule your first appointment.
If you are interested in receiving services via video therapy (telehealth) in addition to, or instead of face-to-face with Dr. Neuger, please be sure to read “Telehealth with Dr. Neuger.” Following the recommendations in that document will give you the greatest opportunity to have a smooth and successful online treatment experience.
